About the event
This event will support delegates through the Implementation of PSIRF. It will focus on the work required in the preparation guide, with expert insight from the team that worked with NHS England to develop the tools and guidance.
This course will give attendees a safe space to think through some of the challenges.
Content will include:
Orientation
- Implementation plan
- Getting buy-in
- Creating a stakeholder list and engagement plan
- Programme management
- Agreeing on the ambition for PSIRF
Delegates will receive:
- a template implementation plan
- briefing papers to start the process of engagement
- example stakeholder lists and engagement plan
- a template programme management structure
- an approach to agree on the organisational ambition for PSIRF
Diagnostic and discovery
- What is being done to support open and transparent reporting?
- How do you engage and involve those affected by patient safety incidents?
- What is being done to support the development of a just culture?
- What is your incident response capacity, and what are your training needs?
- How do you use learning from incident responses to inform improvement?
- What do you need to do next?
Delegates will receive:
- diagnostic tools to assess progress with open and transparent reporting, engaging and involving patients, families and staff, and developing a just culture.

Governance and quality monitoring
- Develop processes for incident response decision making
- Define how the system's effectiveness will be monitored
- Develop processes for reporting cross-system issues
- Define how PSIRF implementation will be monitored

- Governance and decision-making maps, for example, term of reference and reporting and escalation lines
- Example metrics to use to monitor the effectiveness
- Escalation of cross-system issues
Patient Safety Incident Response Planning
- Mapping services
- Examine patient safety incident records and safety data
- Describe the safety issues revealed by the data
- Identifying work is underway to address contributory factors
- Agree how you intend to respond to issues listed in your patient safety profile
Delegates will receive:
- A template on how to map services
- Example patient safety profile
Curation and agreement of policy and plan
- Populate the policy and plan templates and share them with stakeholders
- Respond to stakeholder's feedback on the draft policy and plan
- Agree on how to manage the transition
- Ensure commitment to delivering required improvement
- Seek policy and plan approval/sign-off and agree on a 'transition date.'

Transition
- Monitoring and management of simultaneous systems
